Estimated Annual Salary: $55,000 - $65,000About the Role:

You will be responsible for conducting monthly phone calls with patients to educate them on their conditions and coordinate treatment plans with physicians' offices as needed. Your expertise in creating personalized care plans, identifying social determinants of health, and empowering patients to take charge of their own wellness will be essential in ensuring the best possible outcomes for our patients.

Key Responsibilities:
  • Patient Coordination: Conduct monthly CCM phone calls with patients to ensure they receive necessary care between office visits.
  • Care Plan Development: Create personalized care plans for each patient, in coordination with practices and providers.
  • Social Determinants Identification: Identify social determinants of health, gaps in care, and eligibility for assistance or other referral services for each patient.
  • Patient Empowerment: Empower patients to take charge of their own wellness and goals in the context of their care plan.
Requirements:
  • 1+ year experience as a Certified or Registered Medical Assistant, with active accreditation required.
  • Technology experience - preferably with multiple EMR/EHR systems, familiar with Microsoft Office, GoogleSuite products, and comfortable learning new software quickly.
  • Highly professional phone demeanor, compassionate tone, quick thinking skills, excellent verbal skills, and exceptional business writing skills.
  • Self-motivated, organized, and able to work independently, with call center and/or triage experience a plus.
