About the Position

We are seeking a highly skilled Hospice Social Work Professional to join our team at Good Samaritan. This is a rewarding opportunity for an individual who is passionate about providing exceptional care and support to patients and their families.

Job Summary

The successful candidate will provide medical social services, bereavement services, and coordinated volunteer activities to terminally-ill patients and their families. They will work closely with the interdisciplinary group to develop comprehensive plans of care and provide ongoing, supportive counseling to patients and their families.

Responsibilities
  • Interpret hospice programs and services for patients and families, review referrals, and initiate psychosocial assessments to identify patient/family needs, resources, and acceptance of hospice.
  • Participate in Interdisciplinary Group (IDG) meetings to assist in the development of comprehensive plans of care, communicate and interpret patient/family needs and problems to the IDG.
  • Maintain ongoing, supportive counseling with patients/families, assisting them with difficulties and meeting social and emotional needs relevant to advanced illness, death, and bereavement.
  • Make appropriate referrals to community agencies and/or community resources and advise staff/volunteers regarding services available to patients and families.
  • Provide bereavement counseling and address high-risk needs by assessing timely ongoing assessment of individual social work, bereavement, and/or volunteer needs.
  • Evaluate the effectiveness of interventions, adapting the plan of care accordingly and demonstrate timely, accurate, and appropriate documentation of social work, bereavement, and/or volunteer contacts.
Requirements
  • A minimum of a Bachelor of Science in Social Work from an accredited school of social work.
  • At least one year of experience in a healthcare setting.

Salary

The estimated annual salary for this position is $65,000 - $80,000 based on location and experience.
