Home Health Speech Language Pathologist

VNA Health Group is seeking a skilled Speech Language Pathologist to provide high-quality care to patients in their homes or community settings.
Responsibilities- Evaluate and assess clients for speech and language therapy needs, developing and implementing personalized plans of action in collaboration with physicians.
- Document all changes in client condition and update care plans accordingly.
- Identify counseling needs and plan appropriate teaching strategies.
- Monitor quality and appropriateness of services provided, adhering to agency infection control and safety policies.
- Meet the organization's policy on absenteeism and appearance.
- Licensed Speech-Language Pathologist with CCC-SLP certification and Master's degree in Speech-Language Pathology.
- 2-3 years of prior clinical experience and community-based experience preferred.
- Strong computer skills and effective communication and interpersonal skills.
- Valid NJ driver's license.
