Providence is seeking a skilled Registered Nurse to join our Home Health team. As a Home Health RN, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with an interdisciplinary team to develop and implement personalized care plans.
Key Responsibilities
- Conduct comprehensive assessments of patients' physical, emotional, and social needs
- Develop and implement individualized care plans to address patients' needs and goals
- Provide direct patient care, including wound care, medication management, and education
- Collaborate with patients, families, and healthcare teams to ensure seamless care transitions
- Document patient progress and outcomes in accordance with regulatory requirements
Requirements
- Current California Registered Nurse License
- 1 year of professional nursing experience, preferably in home health or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Basic computer skills and proficiency in electronic medical records
