Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will be responsible for providing exceptional patient care, administering medications, and collaborating with healthcare professionals to develop and implement care plans.

Key Responsibilities

  • Develop and Maintain Nursing Services Objectives
    • Develop and maintain nursing services objectives, standards of nursing practices, and policy and procedures.
  • Assist Director of Nursing
    • Assist the Director of Nursing (DON) with resident care policies and work with the administrator in executing policies.
  • Preventative Medicine Practices
    • Responsible for preventative medicine practices for residents.
  • Nursing Services Coordination
    • Assist in coordinating nursing services with other resident-related services provided within and outside of the facility.
  • Individualized Care Plans
    • Responsible for the development of individualized care plans for each resident.
  • Resident Record Evaluation
    • Evaluate resident records to assure accuracy, care plans are current and complete, and residents are receiving optimal nursing care.
  • Lab and Diagnostic Services
    • Monitor labs, x-rays, dental, optometry, podiatry services, hearing services, and rehabilitative services with other providers.
  • Communication and Collaboration
    • Effectively communicate with nursing staff members.
  • Social Services Coordination
    • Help to conduct formal social service assignments.
  • Discharge Planning
    • Help with formulating and coordinating interdisciplinary discharge plans for residents.
  • Resident Advocacy
    • Assure successful new admissions and transfers of residents both within the center and to outside agencies.
  • Professional Relationships
    • Establish and maintain meaningful, appropriate, professional relationships with residents/family members in order to effectively assist the client in attaining their goals.
  • Nursing Care and Records
    • The total nursing care of residents and the accuracy of the nursing care records.
  • Individual Plan of Care
    • Ensure individual plan of care is followed.
  • Nursing Services Coordination
    • Coordinating nursing services for the residents in the nursing home with other resident care services provided both within and outside of the nursing home.
  • Communication with Physicians
    • Communicate with resident's physician when there is a change in condition.
  • Resident Advocacy
    • Serve as a resident advocate.

Requirements

  • Licensure and Certification
    • Must have an unrestricted license with the State of Minnesota as a Registered Nurse, and maintain current unencumbered licensure.
  • Knowledge and Skills
    • Thorough knowledge of state and federal regulations regarding long-term care.
  • Safety and Communication
    • Knows and practices good safety habits.
  • Interpersonal and Communication Skills
    • Strong communication and interpersonal skills.
  • Work Environment
    • Ability to work in a fast-paced environment.
  • Teamwork and Independence
    • Ability to work independently and as part of a team.

Compensation

$41 per hour
