Job Summary:

The LPN Care Coordinator is a key member of our nursing team at Villas at Robbinsdale, responsible for assisting the Director of Nursing and Nurse Manager in the management and leadership of our skilled nursing unit. This role requires a strong understanding of nursing processes and a ability to communicate effectively with residents, families, and healthcare professionals.

Key Responsibilities:

  • Assist in the evaluation of available resources to ensure quality outcomes of care
  • Assist in the direction and implementation of the nursing process, including systematic data collection and development of care plans
  • Coordinate the management of resident care through a well-defined care delivery system
  • Collaborate with the Director of Nursing and Nurse Manager to ensure effective communication and coordination of care
  • Participate in quality improvement initiatives to enhance resident care and satisfaction

Requirements:

  • Current LPN registration in the state of Minnesota
  • Successful completion of or seeking certification in Cardiopulmonary Resuscitation and emergency care for the healthcare provider
  • Ability to communicate verbally and in writing when coordinating resident care
