The LPN Care Coordinator is a key member of the nursing team at Villas at Robbinsdale, responsible for assisting the Director of Nursing and Nurse Manager in the management and leadership of a specific nursing unit.
Key Responsibilities:
- Assist in the evaluation of available resources to ensure quality outcomes of care
- Assist in the direction and implementation of the nursing process, including systematic data collection, care plan development, and implementation
- Coordinate the management of resident care through a well-defined care delivery system
- Collaborate with the Director of Nursing and Nurse Manager to ensure effective communication and coordination of care
Requirements:
- Current LPN registration in the state of Minnesota
- Successful completion of or seeking certification in Cardiopulmonary Resuscitation and emergency care for the healthcare provider
- Ability to communicate verbally and in writing when coordinating resident care
