Job Summary

CVS Health is seeking a skilled Case Manager RN to join our team. As a Case Manager RN, you will play a critical role in delivering high-quality patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities
  • Provide telephonic case management services to patients and their care teams, ensuring timely and effective communication and coordination of care.
  • Apply clinical judgment to assess patient needs and develop personalized care plans that address complex health and social indicators.
  • Collaborate with multidisciplinary teams, including clinical colleagues, supervisors, and Medical Directors, to overcome barriers and achieve care goals.
  • Utilize case management processes and protocols to ensure compliance with regulatory and company standards.
  • Engage patients in their care through motivational interviewing and other evidence-based strategies.
  • Identify and escalate patient needs as necessary, following established guidelines and protocols.
Requirements
  • 5+ years of experience as a Registered Nurse, preferably in oncology or a related field.
  • Unrestricted RN license in the state of residence, with ability to obtain multi-state/compact privileges.
  • 1+ year of experience documenting electronically and using keyboard and multiple computer screens.
  • 1+ year of experience in oncology, behavioral health, maternity, transplant, or medical/surgical nursing.
Preferred Qualifications
  • 1+ year of case management experience or discharge planning, nurse navigator, or nurse care coordinator experience.
  • 1+ year of experience in utilization review.
  • CCM and/or other URAC recognized accreditation.
  • 1+ year of experience with MCG, NCCN, and/or Lexicomp.
  • Bilingual, preferably Spanish.
Education

Diploma or Associates Degree in Nursing or higher.

Compensation

The typical pay range for this role is $54,116.80. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors, including experience, education, geography, and other relevant factors.

This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above.
