RN Care Coordinator

As a RN Care Coordinator - Nursing Facilities Specialist, you'll play a vital role in our Integrated Care Model by providing essential information about member needs and advocating for the best possible care available.
Our team is dedicated to helping members achieve their healthcare goals, working closely with nursing facilities to ensure they receive the right services to meet their needs. This field-based position requires travel in local areas, approximately 75% of the time, based on business needs.
Key Responsibilities:
- Assess, plan, and implement care strategies tailored to individual member needs, directed toward the most appropriate, least restrictive level of care.
- Identify and initiate referrals for member transition to community resources and provide assistance with social service programs, including financial, psychosocial, community, and state supportive services.
- Manage the care plan throughout the continuum of care as a single point of contact.
- Communicate with all stakeholders for required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
- Advocate for members and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.

Requirements:
- 2+ years of experience working within the community health setting or in a healthcare role
- Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and ability to use multiple web applications
- Reliable transportation and the ability to travel in the assigned region to visit Medicaid members in their homes and/or other settings
- Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI)
- 1+ years of experience with long-term care services and support, Medicaid or Medicare
