Field RN Care Coordinator for Nursing Facilities

We are seeking a skilled RN Care Coordinator to join our team in a field-based position. In this role, you will play a vital part in an Integrated Care Model by gathering and sharing member information, advocating for optimal care, and ensuring access to necessary services.
Key Responsibilities- Assess, plan, and implement personalized care strategies for members, aiming for the most suitable and least restrictive level of care.
- Identify and initiate referrals for members transitioning to community-based care, as well as provide assistance with social services, including financial, psychosocial, community, and state supportive services.
- Manage the care plan throughout the continuum of care, serving as a single point of contact.
- Communicate with all stakeholders to ensure quality coordinated care and services are provided promptly to members.
- Advocate for members and their families as needed to ensure their needs and preferences are represented and supported by the healthcare team.
You will visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices, approximately 75% of the time based on business needs.
