Job Title: Medical Assistant - Community Health Center Professional
Job Summary:
The Medical Assistant - Community Health Center Professional plays a vital role in supporting medical providers and ensuring patients receive high-quality, comprehensive care in a community health setting. This position encompasses clinical and administrative duties, emphasizing patient-centered care, removing barriers to positive health outcomes, and addressing social determinants of health.
Key Responsibilities:
- Patient Care & Clinical Support
- Room patients and perform medical triage, including obtaining vital signs, medical history, and assessing the purpose of the visit.
- Administer and document preventive care measures, such as vaccinations and diagnostic tests, according to clinic protocols.
- Manage medication refill requests as directed by providers.
- Contact patients to schedule planned care visits and provide appropriate education and guidance during those interactions.
- Referral & Social Support Coordination
- Complete and track patient referrals per provider requests, ensuring follow-up care is arranged and outcomes are tracked.
- Identify and address social determinants of health needs, such as transportation, food access, and other support services, connecting patients to both internal and external resources.
- Provide care coordination, advocacy, and evaluation for patients' social service's needs, facilitating access to programs offered within the clinic or through external partnerships.
- Follow up with patients using various methods, including phone calls, home visits, and digital communication, ensuring barriers to care are addressed.
- Patient Advocacy & Relationship Building
- Establish and maintain trusting relationships with patients to ensure ongoing navigation and coordination of care.
- Serve as a patient advocate by providing guidance in navigating healthcare and social services systems, including pharmaceutical assistance programs.
- Maintain up-to-date knowledge of area resources and help patients access relevant services that can improve their well-being.
- Administrative Support
- Document patient encounters, assessments, and care coordination activities accurately in the electronic medical record (EMR).
- Assist in the call center by answering phone calls, scheduling appointments, and relaying messages to clinical staff.
- Work closely with providers and healthcare team members to ensure patient needs are met effectively and efficiently.
- Participate in maintaining a collaborative, positive work environment focused on patient-centered care.
Requirements:
- Previous experience working as a Medical Assistant in a clinical setting.
- Strong interpersonal and communication skills, with the ability to build and maintain trust with patients.
Preferred Qualifications:
- Community Health Worker (CHW) Certification or willingness to obtain certification.
- Experience working in a Federally Qualified Health Center (FQHC) or similar community health setting.
- Knowledge of social service resources and community-based programs.
Physical and Time Requirements:
- Ability to lift 25 lbs and perform duties associated with clinic and office settings.
- Flexible schedule based on clinic hours, which may include evening and weekend activities.
- Tasks occur in a fast-paced environment, requiring the ability to multitask and prioritize effectively.
