Job Title: Home Health Certified Occupational Therapy Assistant

Alternate Solutions Health Network LLC is seeking a skilled and compassionate Home Health Certified Occupational Therapy Assistant to join our team in Dayton, Ohio.

About the Role:

We are looking for a highly motivated and experienced Occupational Therapy Assistant to provide high-quality care to our patients in their homes. As a Home Health Certified Occupational Therapy Assistant, you will work as part of an interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.

Responsibilities:
  • Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
  • Assess patient conditions and notify the Occupational Therapist of any changes.
  • Develop and implement plans of care that promote patient independence and well-being.
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care.
  • Document patient progress and maintain accurate records.
Requirements:
  • Current license as an Occupational Therapist Assistant (OTA) in the state of employment.
  • Minimum one year of experience as an OTA in an acute care setting.
  • Home care experience preferred.
  • Valid driver's license and auto insurance.
