Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health Care team. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Responsibilities
  • Provide direct and indirect nursing care to patients in their homes, under the supervision of a Registered Nurse (RN)
  • Implement and evaluate patient care plans, in collaboration with the RN, to meet the physical, psychological, and social needs of patients
  • Administer medications, perform vital signs, and collect specimens as ordered by physicians
  • Provide patient education and support to promote independence and self-care
  • Collaborate with the patient, family, and healthcare team to develop and implement a comprehensive care plan
  • Document patient progress and care provided in the patient's medical record
Requirements
  • Graduate of an LPN school
  • Current licensure as a Practical Nurse in the state of West Virginia or neighboring state
  • Valid driver's license and minimum auto insurance
  • Current CPR certification
  • One year of home care nursing experience preferred
Working Environment

As a Home Health LPN, you will work in a fast-paced, dynamic environment, providing care to patients in their homes. You will be required to travel to various locations, including patient homes, clinics, and hospitals. You will also be expected to work independently, with minimal supervision, and make sound, independent judgments based on scientific principles.

Physical Requirements

The physical demands of this position include frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling. You must be able to lift, move, and position patients of all weights, with varying amounts of assistance, on a regular and recurring basis.
