About the Role

We are seeking a compassionate and dedicated Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing high-quality care and services to our residents and their families.

Responsibilities
  • Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Develop and implement Individualized Service Plans (ISPs) to ensure each resident receives personalized care and support.
  • Participate in the development of care plans and monthly updates to ensure residents receive the best possible care.
  • Collaborate with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
  • Respond to the dining needs of residents and guests, maintaining Sunrise hospitality and service standards.
Qualifications
  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude and flexibility to perform various duties in service to residents.
  • High School diploma/GED accepted and may be required per state regulations.
  • Previous experience working with seniors preferred.
  • Ability to make choices, decisions, and act in the resident's best interest.
