Job Summary

We are seeking a highly skilled and experienced RN/LVN MDS Coordinator to join our team at Regency Integrated Health Services. The successful candidate will be responsible for coordinating the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of resident assessments, and communicating effectively with the interdisciplinary team.

Key Responsibilities
  • Coordinate the RAI process to ensure accurate and timely completion of resident assessments
  • Communicate effectively with the interdisciplinary team to ensure seamless care coordination
  • Ensure assessments accurately reflect the physical, mental, and psychosocial status of each resident
  • Document and report services provided and assessment accuracy
  • Participate in the pre-admission process to ensure essential information is obtained from the referral source
  • Ensure accurate and timely completion of all Medicare/Medicaid case-mix documents
  • Collaborate with the facility Director of Rehab to ensure the most appropriate assessment reference date is utilized
  • Track Skilled customers utilizing Case Management Tools to determine continued and appropriate Medicare/Managed Care eligibility and benefit period
  • Communicate with Regional Care Management Specialist, Business Office, and external Case Managers as required
Requirements
  • Graduate of an accredited school of nursing with current R.N. or L.V.N. licensure
  • Minimum of two years health care experience, with experience in MDS completion, reimbursement, clinical resource utilization, and/or case management highly desirable
  • Competency with standard office software applications and software applications related to MDS/RAI processes
  • High initiative and ability to efficiently and effectively lead interdisciplinary teams and coordinate and manage RAI process
