Brookdale Senior Living is seeking a highly skilled and dedicated MDS Coordinator LVN to join our team. As a key member of our healthcare team, you will be responsible for conducting and coordinating the development and completion of resident assessments, ensuring compliance with state and federal regulations.

Key Responsibilities:
  • Coordinate the development and completion of resident assessments, including the implementation of CAAs and Triggers.
  • Evaluate each resident's condition and pertinent medical data, developing and implementing procedures for newly admitted residents.
  • Ensure timely completion and transmission of assessments, assisting community directors and supervisors in scheduling resident assessment/care plan meetings.
  • Develop and maintain written policies and procedures governing resident assessments and care plans.
  • Participate in quality assurance programs, monitoring QI and QM reports to ensure corrective action is taken when necessary.
Requirements:
  • Nursing degree from an accredited college or university.
  • Minimum of two years of experience as a supervisor in a hospital, nursing care community, or related healthcare facility.
  • Knowledge of general, rehabilitative, and restorative nursing and medical practices, procedures, regulations, and guidelines governing long-term care.
