Overview

Brookdale Senior Living is committed to providing exceptional care and services to our residents. We are seeking a highly skilled and dedicated MDS Coordinator LVN to join our team.

Responsibilities
  • Conduct and coordinate the development and completion of resident assessments, ensuring compliance with state, federal, and company guidelines.
  • Oversee the generation of Minimum Data Set (MDS) for each Medicare patient and transmit required data within the mandated timeframe.
  • Coordinate the development of written plans of care for each resident, identifying problems/needs, care requirements, and responsible professionals.
  • Ensure timely completion and transmission of MDS, maintaining and updating policies and procedures governing resident assessments and care plans.
  • Develop and implement quality assurance programs for resident assessments and care plans, monitoring QI and QM reports to address potential issues.
  • Assist with discharge planning, participating in functions and providing reports as needed.
  • Participate in community surveys and provide leadership in committees, including Interdisciplinary Care Plan Team and Quality Assessment and Assurance.
  • Develop and conduct in-service training classes on assessment skills and techniques, collaborating with the In-service Director/Educator.
Qualifications
  • Nursing degree from an accredited college or university.
  • Minimum two years of experience as a supervisor in a hospital, nursing care community, or related healthcare facility.
  • Minimum six months of training experience in rehabilitative and restorative nursing practices.
  • Knowledge of general, rehabilitative, and restorative nursing and medical practices, regulations, and guidelines governing long-term care.

Work Environment

The MDS Coordinator LVN will work in a fast-paced, dynamic environment, interacting with residents, colleagues, and vendors. The role requires standing, walking, sitting, and use of hands and fingers. Possible exposure to communicable diseases and infections is a risk. Ability to lift up to 50 pounds is required.
