Nursing Care Specialist

At Interim HealthCare of the Twin Cities, we offer a comprehensive range of home care services to support individuals in maintaining their independence and dignity. As a RN Case Manager, you will play a vital role in delivering high-quality patient care within the comfort of their own homes.
Salary and BenefitsWe are pleased to offer a highly competitive salary range of $84,905 - $101,233 per year, along with a generous hiring bonus of $6,000. In addition, our comprehensive benefits package includes:
- A company car program and mileage reimbursement
- Paid time off (PTO), holiday pay, and 401(k) retirement plan
- Medical, dental, and vision insurance, including an HSA with company contribution
- Clinical license reimbursement and CEU reimbursement up to $400 per year
- Scrubs allowance and opportunities for professional growth and development
This is a fantastic opportunity to work as a RN Case Manager in our Southeast Twin Cities home health team. You will be responsible for performing assessments and procedures, developing working relationships with discharge planners and social service workers, and participating in ongoing interdisciplinary assessment of patients. A key aspect of this role is visiting clients in their homes to evaluate the quality of care being provided and reviewing clinical notes.
To succeed in this position, you will need to have:
- Graduate of an accredited school of nursing and current licensure as a Registered Nurse in the State of MN
- Two years of current nursing experience and one year of case management experience preferred
- Reliable transportation, valid drivers' license, and proof of auto insurance
- CPR certification
