Job Title: Area Support Manager

CTW Home Health LLC is seeking a highly skilled Area Support Manager to join our team. As a key member of our Patient Navigator Team, you will play a critical role in ensuring the delivery of high-quality patient care services.

Job Summary:

The Area Support Manager will be responsible for facilitating the scheduling and coordination of a multidisciplinary plan of care, working closely with treating therapists, patients, and families to ensure seamless care delivery. This role requires strong communication and interpersonal skills, as well as the ability to promote the mission, vision, and values of our organization.

Key Responsibilities:
  • Ensure a quality patient experience by meeting or exceeding set goals, objectives, and metrics of the Patient Navigator Team.
  • Prioritize patient scheduling to maximize available benefit and ensure efficient use of therapist time.
  • Assess patient wait lists to identify patients who can be transferred to an active case and review current therapist caseloads to ensure optimal clustering of patients.
  • Assist with onboarding, orientation, and support of newly hired Patient Navigators and participate in biweekly Therapy Productivity Review sessions for assigned therapists.
  • Maintain accurate caseloads and patient information, ensuring routine review of mapping software to ensure efficient clustering and potential new patient assignment from Wait List.
  • Provide detailed information to Supervisors to assist with assignment of admissions, evaluations, or termination of services.
  • Assist therapists with scheduling of evaluations, re-evaluations, visits, and missed visits as necessary.
  • Educate parents and coordinate with Child Protective Services (CPS) caseworkers as needed to ensure quality and continuity of care.
  • Notify insurance providers as needed when members require education for non-compliance.
  • Participate in daily Area Support Team huddles to review pertinent issues, address concerns, questions, etc., and foster a Team spirit and culture of caring for all Team members.
Requirements:
  • Competencies: Attention to Detail, Customer Focused, Problem-Solving, Interpersonal Savvy, Time Management, Stress Management, Excellent oral and written communication, Organization Skills.
  • Education and Experience: Bachelor's degree or equivalent, or minimum of four (4) years of hands-on related experience and/or training; or equivalent combination of education and experience.
