Job Summary

We are seeking a highly motivated and compassionate Case Manager to join our team at the Latin American Youth Center. As a Case Manager, you will play a critical role in supporting the safety, well-being, and stability of youth in our housing programs. You will work closely with youth, families, and community stakeholders to provide wrap-around services and ensure that our participants have the skills and resources needed to achieve their goals.

Key Responsibilities
  • Develop and implement individualized case plans to address the unique needs and goals of each youth
  • Provide guidance and support to youth in areas such as education, employment, and life skills
  • Collaborate with community stakeholders to identify and access resources and services that support youth success
  • Conduct home visits and apartment inspections to assess the safety and well-being of youth and their families
  • Enter case notes and other data in various databases to track progress and outcomes
Requirements
  • Bachelor's degree in Social Work or related field
  • Experience working with at-risk youth and families
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Fluency in English and Spanish required
