Job Title: Registered Nurse - Nurse Family Partnership

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Nurse Family Partnership (NFP) Nurse. As a key member of our community health team, you will work closely with first-time parents and their families to provide comprehensive home visiting services and support.

Job Summary:

The NFP Nurse will provide coaching, education, and support to first-time parents and their families, focusing on their physical, emotional, and social well-being. You will work in partnership with our clients to establish goals and develop strategies to achieve them, while also connecting them with community resources and services.

Key Responsibilities:
  • Provide home visiting services to first-time parents and their families, focusing on their physical, emotional, and social well-being.
  • Develop and implement individualized plans to support clients' goals and outcomes.
  • Connect clients with community resources and services, including healthcare, education, and social services.
  • Collaborate with other healthcare professionals and community agencies to ensure comprehensive care and support.
  • Participate in ongoing professional development and training to maintain expertise in NFP model and best practices.
Requirements:
  • Bachelor's degree in nursing (BSN) or an Associate's degree in nursing (ASN) with the expectation to begin the path to BSN within 5 years of employment.
  • Current Indiana Registered Nurse (RN) license in good standing or the ability to obtain prior to start date.
  • Valid driver's license, up-to-date auto insurance, and a good driving record.
  • Ability to travel to Denver, CO for in-person education sessions.
  • Current CPR certification.
