Job Summary

We are seeking a skilled Mental Health Case Manager II to join our team. The successful candidate will facilitate the implementation of individualized Service Plans, providing client advocacy services and case management strategies. This is a rewarding opportunity to make a difference in the lives of our clients and their families.

Key Responsibilities
  • Conduct strengths-based, culturally competent, and family-centered mental health rehabilitation services and/or case management assessments.
  • Provide rehabilitative services to clients and/or their families, including assistance in restoring or maintaining functional skills, daily living skills, social skills, and linking up to needed community services and support resources.
  • Complete Service and Coordination Plans, progress notes, and other required documentation within two to three business days.
  • Provide outreach, advocacy, and rehabilitative services needed for ongoing cases and in crisis intervention.
  • Work with the treatment team to provide coordinated care services to clients and their families, addressing client needs and advocating for stabilizing services and connecting the consumer to beneficial resources within the community.
Requirements
  • Bachelor's degree in a related field or two years of experience with chronically mentally ill populations and/or in social services.
  • Experience working with a culturally diverse clientele and staff.
  • Ability to work as part of a multidisciplinary team.
  • Chemical dependency experience preferred.
  • Excellent written and verbal communication skills.
  • Familiarity with Windows operating system, MS WORDS, EXCEL, email, etc.
  • Bilingual skill in the appropriate language for the Clinics' clients is highly preferred.
