Job Summary:

Community Connections, Inc. is seeking a compassionate and dedicated Registered Nurse to join our Day Center team. As a key member of our healthcare team, you will play a vital role in supporting individuals with disabilities by providing essential healthcare services.

Responsibilities:

  • Dispense and document consumer medications in a timely and accurate manner.
  • Participate in individual case record reviews to ensure comprehensive care.
  • Conduct regular medication reviews to identify potential issues.
  • Review consumer health concerns with the Clinical Team to ensure seamless care coordination.
  • Monitor and record vital signs and weights to track consumer health.
  • Communicate effectively with healthcare professionals and service coordinators to ensure comprehensive care.
  • Perform other assigned job-related duties as needed.

Requirements:

  • Must be fully licensed with the Massachusetts Nursing Board.
  • Bachelor's Degree from a four-year college or university (preferred).
  • Minimum of 1 year experience in a community health setting.
  • Minimum of 1 year experience in a program for individuals with developmental disabilities.
  • Valid driver's license and access to a motor vehicle with a satisfactory driving record.
  • Excellent verbal communication and interpersonal skills.
  • Ability to work independently and collaboratively.
  • Proficiency in computer and word processing skills.
