Job Title: Chronic Care Registered Nurse

We are seeking a skilled and compassionate Chronic Care Registered Nurse to join our team at TalentBurst, Inc.

Job Summary:

The successful candidate will be responsible for delivering high-quality patient care through the nursing process, including assessment, diagnosis, planning, implementation, and evaluation. This role requires a strong understanding of chronic care principles and the ability to work collaboratively with other healthcare professionals to achieve desired patient outcomes.

Responsibilities:
  • Deliver patient care through the nursing process, including administering medications and treatments, interpreting diagnostic tests, and formulating a plan of care.
  • Collaborate with other healthcare providers to ensure effective and efficient patient care delivery.
  • Provide education to patients and/or significant others, while maintaining confidentiality and adhering to HIPAA guidelines.
  • Document and chart observations and other data related to the clinical condition of patients.
  • Communicate effectively with healthcare workers and perform all required and related duties.
Requirements:
  • 1 year of Nephrology Nursing experience in the last two years as a RN.
  • Chronic hemodialysis experience required.
  • Graduated from an accredited school of professional nursing.
  • Licensure appropriate to the state the RN is working in and the appropriate certifications to the position and facility.
  • Minimum 1 year previous job experience.
  • Proficient oral and written skills.
  • Current CPR certification.
  • Annual Physical.
  • Two professional references from current/former supervisors.
Physical Requirements:

The individual may be required to pull up or reposition patients, lift supplies, place equipment on supports, push bed or chair through the facility. The individual must be able to bend over to perform certain duties and maneuver throughout the facility in response to any facility emergency.

The individual must be able to interpret environmental input, which requires visual and auditory skills. In the event of a need to evacuate the building, heavy lifting of patients may be required.
