Role Summary:
The Practical Nursing Specialist plays a crucial role within the healthcare team, providing nursing care to patients in their designated unit. This position involves implementing the nursing process from patient admission to discharge, under the guidance and collaboration with Registered Nurses. The Practical Nursing Specialist is responsible for effective communication with healthcare team members, ensuring that patient care needs are met efficiently.
Key Responsibilities:
- Gather comprehensive health status information relevant to patient concerns systematically.
- Assist in creating personalized Care Plans based on achievable goals, documenting thoroughly in patient records.
- Exhibit strong organizational skills to prioritize patient care needs effectively.
- Coordinate resources and personnel to execute the Care Plan efficiently.
- Administer treatments and medications as prescribed by physicians, adhering to the Care Plan.
- Assess patient responses to treatments and modify the Care Plan as necessary.
- Document all patient care activities, medication administration, and any changes to the Care Plan in accordance with established policies.
- Provide education to patients and their families based on the Care Plan and maintain accurate documentation.
- Facilitate effective communication with healthcare team members to ensure seamless care delivery and report any changes in patient conditions.
- Support safe and timely patient transitions, including admissions, transfers, and discharges.
- Adhere to hospital policies and regulatory standards in all aspects of nursing practice.
- Respect patient rights, dignity, and confidentiality at all times.
- Engage actively in unit meetings and utilize the chain of command for issue resolution.
Qualifications:
Must hold a current nursing license in the State of Maryland and be a graduate of an accredited nursing program. Strong interpersonal and communication skills are essential for success in this role.
