As a community teaching hospital, we focus on assisting individuals in managing chronic health conditions, including but not limited to diabetes, hypertension, and pulmonary diseases.
Core Responsibilities:
The Practical Nursing Specialist plays a vital role within the healthcare team, providing nursing care to all patients in their designated unit/area. This role involves implementing the nursing process from patient admission through discharge, in collaboration with Registered Nurses.
The Practical Nursing Specialist will work closely with other healthcare professionals, delegate tasks to support staff, and ensure that patient care needs are effectively met.
It is essential for the Practical Nursing Specialist to exercise sound judgment in patient care and to be accountable for the quality of care provided. Staying informed about the Nurse Practice Act is crucial for this role.
Key Duties:
- Gather comprehensive health status data relevant to the patient's presenting condition in a systematic manner.
- Assist in creating a personalized Plan of Care with realistic and measurable goals, ensuring thorough documentation in the patient's record.
- Demonstrate strong organizational skills to prioritize patient care needs effectively.
- Coordinate and manage available resources and personnel to execute the Plan of Care.
- Administer treatments and medications as prescribed by physicians, following the established Plan of Care.
- Assess patient responses to treatments and modify the Plan of Care as needed.
- Document all aspects of patient care, including medication administration and any changes to the Plan of Care, in accordance with approved policies.
- Provide education to patients, families, and significant others based on the Plan of Care and ensure proper documentation.
- Communicate effectively with other healthcare team members to guarantee continuity and coordination of care.
- Facilitate safe and timely patient transitions, including admissions, transfers, and discharges.
- Adhere to hospital policies and regulatory standards in all aspects of nursing practice.
- Respect the rights, dignity, and confidentiality of all patients.
- Participate actively in unit staff meetings and utilize the chain of command for issue resolution.
- Support and align with the mission, vision, values, and policies of the University of Maryland Medical System.
- Perform additional duties as assigned to assist the Clinical Manager and other team members.
Qualifications:
- Current licensure in the State of Maryland.
- Graduation from an accredited nursing program.
- Completion of American Heart Association BLS certification.
- Strong interpersonal, communication, and supervisory skills.
