Case Manager/Care Coordinator

Volunteers of America-Greater New York (VOA-Greater New York) is a leading anti-poverty organization dedicated to ending homelessness in the New York area by 2050. We operate over 60 programs across New York City, Northern New Jersey, and Westchester County, providing emergency, transitional, and permanent housing services to individuals and families in need.
Job Summary:The Case Manager/Care Coordinator is a critical member of our team, responsible for assisting homeless individuals with substance use disorders gain access to treatment, benefits, and housing. This role involves conducting outreach activities, managing a caseload of enrolled clients, and working with a Peer Navigator to ensure clients are connected to appropriate services.
Key Responsibilities:- Conduct outreach and engagement visits to Wellness Centers, Safe Havens, Shelters, mental health, and substance use treatment programs.
- Manage the client intake and assessment process, producing a comprehensive assessment of client service needs.
- Develop and update individual service plans with clients, reflecting assessment findings and goals.
- Meet regularly with clients to assist them in obtaining needed services and benefits.
- Provide supportive counseling to strengthen clients' ability to achieve behavior change and other goals.
- Promote a successful transition to permanent housing through skills-building, case management, and counseling.
- Document activities in accordance with agency and funder guidelines and procedures.
- Minimum of 60 college credits (BA preferred) plus two years of relevant work experience.
- Familiarity with homelessness, mental health, and/or substance abuse population required.
- Demonstrated ability in verbal and written communication, including basic computer skills.
- Fluency in Spanish or other second language a plus.
- Medical, dental, vision, and prescription coverage.
