We are seeking a highly skilled and experienced Nurse Practitioner or Physician Assistant to join our team as a Transfer Center APP. This is an exciting opportunity to work in a dynamic and fast-paced environment, providing leadership and clinical expertise to our patients and families.
Key Responsibilities- Performs administrative reviews of transfer center requests in real-time to decrease intake to response time.
- Reviews same-day direct admissions in times of high hospital capacity as decided by CARE Hub leadership team.
- Engages clinical teams when transfer requests require multiple services.
- Responsible for conferencing appropriate transfer back patients.
- Serves as an escalation provider for Clinical Expeditor.
- Graduate of ARC accredited Physician Assistant program (PA) or accredited MSN or DNP Nurse Practitioner program for adult care (NP).
- Certification as a PA or NP.
- Current unrestricted license issued by the NCMB (PA) or NCBON (NP) with approval to practice as a PA or NP.
- Minimum 1+ years experience as a Physician Assistant or Nurse Practitioner.
- Able to clearly articulate scope of practice and practices within those guidelines as an NP/PA in the state of North Carolina and within the Duke University Health System (DUHS).
- Able to independently seek out resources and work collaboratively to solve complex problems.
- Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others.
- Able to use sensory and cognitive functions to process and prioritize information, perform health assessments, treatment, and follow-up.
