Job Title: Home Health Physical Therapy Assistant

LHC Group is seeking a skilled Home Health Physical Therapy Assistant to join our team in Cleveland, Ohio.

Job Summary

We are looking for a dedicated and compassionate Physical Therapy Assistant to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapy Assistant, you will work closely with our team of healthcare professionals to develop and implement personalized treatment plans that promote optimal patient outcomes.

Responsibilities
  • Provide exercise instruction and patient education to patients in their homes
  • Assist patients with active and passive exercises, muscle re-education, gait training, and other physical therapy interventions
  • Observe and record patient responses to treatment and report to the supervising Physical Therapist
  • Collaborate with other healthcare team members to ensure seamless care coordination
Requirements
  • Current licensure, registration, or certification to assist in the practice of physical therapy in the state of Ohio
  • Current CPR certification
  • Valid driver's license and access to a reliable vehicle
