Job Summary

Connecticut Children's is seeking a highly skilled Pediatric Social Work Professional to join our team. This role is responsible for providing psychosocial support to patients and families, conducting assessments and evaluations, and collaborating with healthcare teams to ensure comprehensive care.



Key Responsibilities

  • Develop and maintain effective relationships with patients and families to provide support and advocacy.
  • Conduct timely and comprehensive psychosocial assessments and evaluations to identify patient and family needs.
  • Collaborate with healthcare teams to develop and implement patient-centered care plans.
  • Provide crisis intervention and therapy modalities as needed.
  • Document patient information and referrals in a timely and accurate manner.
  • Provide clinical supervision and guidance to peers and students.


Qualifications

Required:



  • Masters in Social Work required.
  • LMSW required within one year of hire.
  • Minimum of 2 years related experience serving the needs of children and families.

Preferred:



  • Bachelor of Science in Social Work.
  • LCSW certification.
  • Basic knowledge and understanding of medical conditions and healthcare experience of specific patient populations.
