This role requires a strong commitment to delivering high-quality patient care and working collaboratively with the healthcare team to achieve positive patient outcomes.
Key Responsibilities- Provide direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
- Implement current nursing practice following a comprehensive assessment and the plan of care.
- Document patient services accurately and timely to reflect the plan of care.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-care plan.
- Participate in coordination of hospice services, reporting identified needs to the interdisciplinary group.
- Use and prepare equipment and supplies effectively, adhering to aseptic technique.
- Provide appropriate pain/symptom management, evaluating patient response to treatments/medications.
- Participate in personal and professional growth and development, as well as organization's quality assessment and performance improvement teams.
- Graduate of an accredited practical nurse or vocational nursing program.
- Three years of nursing experience, with community health/hospice or medical/surgical experience preferred.
- Currently licensed as an LPN/LVN in the State.
- Comply with accepted professional standards and practice.
- Demonstrate good verbal and written communication, and organization skills.
- Possess and maintain current CPR certification.
