Overview:

At St. Luke's Health System, our team of Hospice RN Liaisons strives to deliver exceptional patient experiences by building positive, supportive, and inclusive relationships with hospital staff and providers.

A successful Liaison will have excellent communication skills, be able to establish trusting relationships, and have a genuine passion for caring for end-of-life patients to ensure their needs are met.

This role requires travel within Twin Falls and surrounding areas.

Key Responsibilities:

  • Coordinate and facilitate referrals for end-of-life patients
  • Schedule initial assessments and admissions
  • Establish patients' eligibility for care
  • Review referrals for appropriate placement
  • Coordinate discharge and post-discharge requirements
  • Meet with patients/families to describe our services and educate them on the benefits of hospice care
  • Work closely with providers and staff for other possible referrals

Qualifications:

  • Education: ASN Degree
  • Experience: 1 year of RN experience, preferably in a Liaison role; Home Health, Acute Care, and/or Case Management experience helpful
  • Licenses/Certifications: Current RN License in the State of Practice; current Basic Life Support (BLS) Certification
  • Valid Driver's License and proof of Auto Insurance
