Job Summary

The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. This role involves a collaborative approach to anticipate, integrate, and coordinate patient care plans to maximize the ability to remain in a community setting.

Key Responsibilities
  • Identify at-risk patient populations who require care management services in collaboration with physicians.
  • Serve as a patient/family advocate and act as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
  • Support the mission, values, and management of patient care services at WakeMed.
  • Actively support and incorporate ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
Requirements
  • 2 years of nursing experience or no experience required with preferred education.
  • Diploma in nursing or associate's degree in nursing required, with a bachelor's degree in nursing preferred.
  • Registered Nurse licensure required.
