- Assist with Daily Living Tasks: Assist residents with daily living tasks, such as meal preparation, medication management, and household chores.
- Document Resident Care: Accurately document resident care, including progress notes, medication administration records, and incident reports.
- Communicate with Residents and Families: Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
- Maintain a Clean and Safe Environment: Maintain a clean and safe environment for residents, including their living spaces and common areas.
- High School Diploma or Equivalent: Must have a high school diploma or equivalent.
- Nurse Aide Certification or Equivalent: Nurse Aide Certification or equivalent is preferred.
- Excellent Communication Skills: Must have excellent verbal and written communication skills.
- Ability to Work Independently: Must be able to work independently with minimal supervision.
- Ability to Lift and Move Residents: Must be able to lift and move residents as needed.
- Ability to Work Flexible Hours: Must be able to work flexible hours, including weekends and evenings.
The Care Manager will work in a fast-paced environment, providing care and support to residents in the Assisted Living neighborhood. The team member will be required to stand, walk, sit, use hands to finger, handle or feel objects, tools, or controls, and reach with hands and arms above, below, and at shoulder height. The team member will frequently lift and/or move up to 25 pounds and may be required to assist in the evacuation of residents during an emergency.
