About the Opportunity

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at a residential facility in Brooklyn, New York City. As a Residential LPN, you will play a vital role in providing high-quality healthcare services to adults with developmental disabilities.

Responsibilities
  • Assist the Registered Nurse (RN) in coordinating and following through on provisions of healthcare services
  • Keep progress notes and summarize for quarterly, semi-annual, and annual reviews
  • Provide healthcare to individuals receiving services, including direct hygiene and health instruction
  • Handle control of communicable diseases and infections through identification and assessment
  • Provide first aid to persons in need of immediate care
  • Assist the Physicians in medical examinations, as needed
  • Schedule appointments and follow up on Physicians' recommendations
  • Keep up-to-date and accurate records of examinations, immunizations, illnesses, accidents, and other pertinent data via progress notes
  • Administer medications when appropriate
Qualifications
  • 1+ year of experience as a Licensed Practical Nurse (LPN)
  • Bachelor's Degree
  • NYS Licensed Practical Nurse (LPN) certification
  • CPR certification
  • Experience in the delivery of community-based health services to individuals with developmental disabilities
  • Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices
  • Solid assessment and documentation skills
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
