A leading nonprofit health organization in New York City is seeking a skilled Licensed Practical Nurse (LPN) to join their residential facility in Brooklyn on a full-time basis.

Residential facility providing healthcare services to adults

Patient Population:
Adults requiring community-based health services

Responsibilities:

The LPN will:
Collaborate with the Registered Nurse (RN) to coordinate and deliver healthcare services
Maintain accurate and up-to-date records of patient progress and health information
Provide direct care and health instruction to patients and their families, as needed
Identify and assess communicable diseases and infections, and take necessary precautions
Administer medications and provide first aid as required
Assist physicians with medical examinations and follow up on recommendations
Keep accurate records of patient data, including examinations, immunizations, and illnesses
Perform other related duties as requested by management

Qualifications:
1+ year of experience as a Licensed Practical Nurse (LPN)
Bachelor's degree in a related field
NYS Licensed Practical Nurse (LPN) certification
CPR certification
Experience in community-based health services for individuals with developmental disabilities
Knowledge of equipment used by patients, including wheelchairs and adaptive devices
Strong assessment and documentation skills
Excellent interpersonal and communication skills
Highly organized and detail-oriented individual
