Job Summary:

The Licensed Practical Nurse (LPN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

Essential Job Functions:
  1. Deliver and Coordinate Quality Patient Care: Provide high-quality patient care, in compliance with physician orders.
  2. Continuously Observe and Assess Patient Condition: Continuously observe and assess patient condition and care needs, reporting changes in condition to the supervisor and/or physician as appropriate.
  3. Document All Activities: Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner whenever care is delivered.
  4. Participate in Nursing Care Plan: Participate in the development, implementation, and update of the nursing care plan.
  5. Take Appropriate Nursing Action: Take appropriate nursing action based on assessment and achieves expected outcomes.
  6. Recognize Changes in Patient Needs: Recognize changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.
  7. Accept Responsibility: Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
  8. Provide Infection Control Measures: Provide care utilizing infection control measures that protect both the staff and the patient according to OSHA standards.
  9. Educate Patients and Families: Educate patients and families regarding the disease process, self-care techniques, and prevention strategies, and in meeting the patient's nursing needs.
  10. Maintain Competencies: Maintain knowledge of competencies related to the nursing profession by participating in educational programs, continued education units, internal learning management skills, and skill evaluations.
Requirements:
  • Graduate of an Accredited School of Nursing: Must be a graduate of an accredited school of nursing.
  • Current, Unrestricted State License: Must have a current, unrestricted state license as a Licensed Nurse in the state of practice.
  • CPR Certification: Must have current CPR certification.
  • Demonstrated Proficiency: Must demonstrate proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferences:
  • Recent Experience: Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home Health Experience: Home health experience is preferred.
Other Skills/Abilities:
  • Attention to Detail: Must have attention to detail.
  • Time Management: Must be able to manage time effectively.
  • Effective Problem-Solving: Must have effective problem-solving and conflict resolution skills.
  • Good Organization and Communication Skills: Must have good organization and communication skills.
Physical Requirements:
  • English Language Proficiency: Must be able to speak, write, read, and understand English.
  • Travel: Must be able to travel.
  • Lifting: Must be able to lift 50 pounds.
  • Physical Abilities: Must be able to sufficiently reposition patients and move equipment without assistance.
  • Visual and Hearing Acuity: Must have visual and hearing acuity.
  • Strong Sense of Smell and Touch: Must have a strong sense of smell and touch.

  • Varied Environments: Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
