Pay: $32.75 per hour, with credit for experience.

About the Position

Londonderry Village is committed to providing person-centered, individualized care and services to the elders we serve.

In the LPN position, you will ensure quality nursing care is provided to elders, as well as instruct Nursing Assistants on medical care and issues.

Essential Duties and Responsibilities:
  • Perform various duties to ensure quality nursing care is provided to elders, focusing on maintaining or attaining the highest practical level of functioning.
  • Evaluate elder's condition and analyze information to develop appropriate interventions to prevent decline.
  • Record signs of change in condition and notify appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown, and other changes.
  • Notify appropriate staff of need for restorative nursing evaluation based on change in condition.
  • Administer medications and treatments to elders as per physician order, documenting administration in the electronic health record (EHR).
  • Evaluate elders upon admission to develop baseline Care Plan and further assess to contribute to comprehensive Care Plan.
  • Attend and participate in Care Plan meetings as needed, updating Care Plan as needed.
  • Respond to emergencies, evaluating the condition of the elder, deciding on the type of medical attention required consistent with elder's Advanced Directive, calling appropriate ambulance service if needed, notifying physician when a change in condition occurs, and notifying family or Agent under a Power of Attorney when elder is transferred or admitted to the hospital.
  • Confer with elders and families about care, making referrals to other departments when necessary.
Qualifications:
  • Valid LPN License
  • Strong background in patient/resident care and supervision
  • CPR certification (preferred)
  • Familiarity with EMR and EHR systems
