Child and Family Support Coordinator Position at New Horizon Counseling Center

The **Child and Family Support Coordinator** role at New Horizon Counseling Center is essential in collaborating with a multidisciplinary team to assist families and children facing significant emotional challenges, ongoing health issues, or traumatic experiences. This position is crucial in helping children achieve their optimal level of functioning and well-being by recognizing their strengths and needs, and linking them to available community resources.

**Primary Responsibilities**:

  • Conducting continuous assessments and evaluations to ensure appropriate service delivery for each case.
  • Coordinating and overseeing all services, including medical, mental health, and educational support through cooperative relationships.
  • Providing in-home and community visits for counseling, crisis intervention, referrals, advocacy services, and assistance with essential needs.
  • Working alongside each child and family to formulate care plans and crisis management strategies.
  • Preparing necessary documentation, maintaining records, and adhering to state and city regulations.
  • Engaging in supervisory and case conference meetings, training sessions, and other required gatherings.

**Qualifications**:

  • A Bachelor's Degree is required; a Master's Degree is preferred in areas such as Social Work, Psychology, Education, or related fields.
  • Certification as a CANS-NY Assessor is mandatory within 30 days of employment commencement.

**Skills and Experience**:

  • Minimum of 2 years of relevant human services experience for candidates with a Bachelor's degree, or 1 year for those with a Master's degree.
  • Strong interpersonal, communication, and time management skills are crucial.
  • A collaborative team player with creativity, dedication, and initiative.
  • Willingness to travel and utilize various transportation methods.

**Preferred Attributes**:

  • Familiarity with Community Resources, Medicaid, and entitlement programs.
  • Understanding of Trauma, Child Welfare, or Child Development.
  • Bilingual abilities are considered advantageous.

**Employment Type**: Full Time; **Salary Range**: Starting salaries at $47,000 annually.
