Job SummaryWe are seeking a skilled and compassionate Home Health Visiting Nurse to join our team. As a Home Health Visiting Nurse, you will provide intermittent skilled nursing services to patients in their homes, working closely with other healthcare professionals to ensure seamless care.
Key Responsibilities- Assess and evaluate patient needs, identifying mutually agreed-upon goals and developing individualized patient care plans.
- Provide skilled nursing services, including assessment, evaluation, procedures, teaching, and training activities, as outlined in the patient Plan of Care.
- Communicate patient status and needs to other disciplines, including physicians, Clinical Supervisors, and referring physicians.
- Supervise and instruct non-professional home health aide staff in the provision of personal care to patients.
- Participate in discharge planning, ensuring a smooth transition of care for patients.
- Collaborate with other healthcare professionals to ensure comprehensive care and address patient needs.
- Document patient care plans, progress, and outcomes, maintaining accurate and up-to-date records.
- Perform OASIS time-point assessments and submit recertification paperwork as required.
- Act as a preceptor for new nursing staff, providing guidance and support in their orientation.
- Participate in Process Improvement initiatives, contributing to the development of new policies and procedures.
- Current, unencumbered State professional Nurse License.
- Valid driver's license and good driving record.
- Ability to make sound professional clinical judgment.
- Proficient clinical skills, including assessment, evaluation, and documentation.
- Excellent verbal and written communication skills.
- Proficiency in personal computer use, including email, clinical, word processing, spreadsheet, and presentation software.
- Active CPR Certification (for Florida-based positions).
- One year of experience as a home care professional nurse, with competence in performing home care comprehensive assessments.
