We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at St. David's Healthcare at Home. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working in collaboration with our interdisciplinary team to develop and implement individualized care plans.
Key Responsibilities- Administer skilled nursing care to patients in their homes, following physician orders and the Plan of Care.
- Conduct ongoing assessments to identify the physical, psychosocial, and environmental needs of patients, and document findings in patient medical records.
- Communicate significant findings, problems, or changes in patient condition to the supervising RN and/or Director of Professional Services, and document all findings, communications, and interventions.
- Provide basic and specialized teaching as ordered in the Plan of Care, and based on patient needs, to promote patient education and empowerment.
- Identify and recommend other disciplines needed for quality care of the patient, and collaborate with the healthcare team to ensure comprehensive care.
- Graduate of an accredited LPN/LVN program.
- Minimum of one year experience as an LPN/LVN in a clinical care setting.
- Home care experience preferred.
- Must be licensed as an LPN/LVN in good standing in the state(s) where they will be providing services.
- Meets all additional requirements for hire, including state-specific requirements.
- Current BCLS prior to delivering patient care.
- Reliable transportation and proof of valid automobile liability insurance.
- Must have valid driver's license.
