St. David's Healthcare at Home is seeking a skilled and compassionate Home Health Aide to join our team. As a Home Health Aide, you will play a vital role in providing high-quality care to patients in the comfort of their own homes.
Key Responsibilities:
- Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, transferring, and getting to and from activities and meals.
- Document the delivery of quality care in the patient's clinical record.
- Implement patient care that is appropriately documented; maintain confidentiality at all times.
- Maintain consistent and concise lines of authority and responsibility with effective communication that promotes a positive working relationship with patients, families, and co-workers.
- Identify changes in patient status or needs and communicate to the nurse timely.
Requirements:
- High school diploma or General Education Degree (GED)
- Minimum of one to three months related experience and/or training or equivalent combination of education and experience
- Previous experience in a healthcare setting is preferred
- Reliable transportation and proof of valid automobile liability insurance
- Must have valid driver's license
