Job Title: Service Coordinator

We are seeking a highly motivated and detail-oriented Service Coordinator to join our team at Adelante. As a Service Coordinator, you will be responsible for providing direct client support, completing and reviewing client documentation, and ensuring the quality of client programming.

Key Responsibilities:
  • Provide direct client support through working with clients, providing crisis management, implementing and monitoring therapy programs, and providing staff training.
  • Complete and monitor client Individual Support Plans (ISPs) required.
  • Assure quality of client programming.
  • Ensure that assigned client programs meet or exceed internal and external quality assurance standards.
  • Facilitate client meetings and staff in-services as needed.
  • Monitor funding of assigned clients.
  • Complete and provide funding justification.
Requirements:
  • High School Diploma (or equivalent) required with two years completion of a university program in education, psychology, rehabilitation, or other related field preferred.
  • Experience working with people with disabilities required.
  • Extensive knowledge of therapies (PT, OT, SLP, etc.) and skills training principles preferred.
  • Must have effective written and verbal communication skills.
  • Must have the ability to effectively communicate in English, with internal and external customers of Adelante.
Other Requirements:
  • Must be pass background/fingerprint investigation and pre-employment drug screening.
  • Must possess a valid NM driver's license and proof of auto liability insurance.
  • Must have own reliable transportation and agency insurable driving record.
  • Must have computer experience.
  • Must be able to push/pull and lift 20-75 lbs., frequently.
