Job Overview

At Exceptional Healthcare, we are committed to delivering exceptional patient care and outcomes. We are seeking a skilled Regional Care Coordinator to join our team and contribute to our mission.

Responsibilities

  • Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation.
  • Communicate patient needs to various care team members and follow up accordingly.
  • Manage safe and effective discharge plans upon completion of treatment.
  • Work collaboratively with patients, families, physicians, and nurses to ensure high-quality care.
  • Act as the patient's advocate regarding insurance coverage and financial assistance.
  • Maintain comprehensive clinical records through detailed documentation.
  • Coordinate with third-party payors on a regular basis.
  • Serve as a utilization manager to determine appropriateness of admission, level of care, and continued stay.
  • Maintain a working knowledge of Medicare and private insurance companies to ensure quality of care.
  • Coordinate insurance appeals as needed.
  • Collect and report quality indicators and variance data to the Utilization Committee and/or relevant departments.
  • Facilitate Quarterly Utilization Review Committee meetings.
  • Perform other duties as assigned and accept new tasks as opportunities for growth.

Qualifications

  • Registered Nurse in the state of practice.
  • Certified Case Manager, preferred.
  • 3-5 years of clinical experience, preferably in case management or related fields.
  • Ability to walk and stand 50-80% of the time.
  • Proficient computer skills, including experience with Microsoft Excel.
  • Strong written and verbal communication skills.
  • Excellent clinical assessment and analytical skills.

Schedule

  • 8-hour shifts, Monday through Friday (9AM - 5PM), with flexibility to accommodate organizational needs.
  • Weekend availability may be required.
  • Occasional travel between Arizona facilities may be necessary.
