Key Responsibilities:
1. Engage with families to identify both natural and formal support systems as per the Arizona Model, Child and Family Team (CFT) process, and Regional Behavioral Health Authority (RBHA) guidelines.
2. Lead CFT/Service Plan meetings to develop personalized service plans based on input from CFT members.
3. Ensure families comprehend the behavioral health process and the role of the CFT Facilitator, fostering family involvement and linking them to community resources.
4. Organize and schedule CFT/Service Plan meetings with relevant stakeholders and support systems.
5. Conduct thorough screenings with families post-intake to build rapport, identify critical needs, and facilitate appropriate referrals.
6. Maintain accurate electronic medical records for each case.
7. Execute strength assessments and crisis/safety plans with families in line with RBHA and ADHS standards.
8. Cultivate positive relationships with clients, families, stakeholders, and colleagues.
9. Perform additional duties as required.


Qualifications:
• Minimum age of 21 years
• Bachelor's degree or High School Diploma with at least four (4) years of experience in behavioral health services, or a suitable combination of education and experience
• Compliance with Agency requirements for Behavioral Health Technician
• Valid AZ Driver's License
• AZ Fingerprint Clearance Card or eligibility for one


Population Served:
Demonstrates the necessary knowledge and skills to provide appropriate care for the age group served in the department: Infant & Early Childhood (birth – 5 years).
