Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health Care Specialist. As a key member of our interdisciplinary team, you will provide comprehensive care to patients with complex medical needs, working collaboratively with families, caregivers, and other healthcare professionals to ensure seamless care coordination.

Key Responsibilities
  • Provide high-quality, patient-centered care to medically fragile patients in their homes, schools, or community settings.
  • Develop and implement individualized care plans that address the unique needs of each patient, including feeding challenges, tracheostomies, ventilator support, seizure disorders, and more.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care coordination.
  • Communicate effectively with patients, families, and caregivers to ensure clear understanding of care plans and goals.
  • Document patient progress, including medical history, treatment plans, and outcomes, in accordance with agency policies and regulatory requirements.
  • Participate in ongoing education and training to stay current with best practices in home health care and complex case management.
Requirements
  • Current licensure as a Registered Nurse in the state of Massachusetts.
  • Experience in home health care or complex case management, with a focus on medically fragile patients.
  • Strong clinical skills, including proficiency in nursing skills, critical thinking, and problem-solving.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to adapt to changing patient needs and priorities, with a focus on providing high-quality care in a fast-paced environment.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
