We are seeking a highly skilled and experienced Home Health Clinical Manager to join our team at Providence Home and Community Care. As a key member of our clinical leadership team, you will be responsible for coordinating the delivery of high-quality home health services to our patients.
Key Responsibilities- Lead and manage a team of home health staff RNs to ensure the delivery of exceptional patient care.
- Develop and implement clinical policies and procedures to ensure compliance with regulatory requirements and best practices.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
- Provide education and training to staff on clinical best practices and new technologies.
- Conduct regular quality assurance and performance improvement activities to ensure high-quality patient care.
- Bachelor's degree in a healthcare-related field.
- Current Washington state RN license.
- National Provider BLS certification within 30 days of hire.
- Three years of experience in nursing or rehab field.
- Case management experience.
- Supervisory experience in home health.
