About the Role

We are seeking a highly skilled and experienced RN to fill the position of Patient Care Manager - Hospice. This role requires strong leadership and hospice care expertise to lead our team in delivering exceptional patient care.

Key Responsibilities
  • Leadership and Team Management: Lead, inspire, and motivate our team to provide high-quality patient care, ensuring all staff are oriented and supported to excel in their roles.
  • Patient Care Coordination: Liaise between staff, patients, and the medical community to ensure seamless care delivery and adherence to regulatory requirements.
  • Quality and Compliance: Promote and direct the team to maintain high standards of patient care and safety, engaging in thorough problem resolution and complaint investigation.
  • Staff Development and Education: Provide in-services to staff on regulatory issues, agency policies, and procedures, addressing identified educational needs to promote patient health and safety.
Requirements
  • Education and Experience: Current RN licensure in the state of practice, at least 4+ years of full-time experience as an RN, and current CPR certification.
  • Additional Requirements: Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
