We are seeking a dedicated Holistic Support Coordinator to join our Emergency Service Team at Jewish Family Service Agency.

As a vital member of our team, you will work directly with homeless clients to create personalized case management plans and provide related support services.

Key Responsibilities
  • Conduct thorough client assessments to determine needs and develop tailored case plans.
  • Provide in-home intensive case management to eligible program participants.
  • Ensure timely completion of all required paperwork for clients being assisted.
  • Connect clients with necessary resources, tools, and counseling to achieve their goals.
  • Maintain positive relationships with clients, community agencies, and team members.
  • Document all client interactions, maintain accurate files, and record important information.
  • Promote collaboration and teamwork within the Emergency Services unit.
  • Participate in committee meetings and training sessions focused on community resources.
  • Adhere to strict confidentiality guidelines and report concerns or needs to the Director in a timely manner.
