We are seeking a highly skilled and compassionate Clinical Nurse to join our team at Scioto County Counseling, Inc. as a Patient Care Specialist. In this role, you will be responsible for providing exceptional patient care and nursing services in a fast-paced healthcare environment.
Key Responsibilities- Provide Patient Care and Nursing Services: Deliver high-quality patient care and nursing services, adhering to the guidelines of the state of Ohio Board of Nursing and the policies and procedures of Scioto County Counseling, Inc.
- Assist with Inventory Control: Collaborate with the Medical Clinic Office Manager to maintain accurate and up-to-date inventory control in the health center.
- Maintain a Clean and Safe Environment: Work with other health center staff to ensure a clean, safe, and organized environment for staff and patients.
- Compliance and Policy Management: Assist the Medical Clinic Office Manager in maintaining accurate and up-to-date policies and procedures, protocols, and standing orders, as well as compliance issues required by funding, licensure, and certifying bodies.
- Represent the Department/Agency: Positively represent Scioto County Counseling, Inc. through appropriate interaction, personal appearance, and attitude.
- Patient Confidentiality: Maintain patient confidentiality at all times, as required by 42 CFR and HIPAA.
- Electronic Health Record Management: Use the electronic health record to retrieve relevant information and document care concisely.
- Clinical and Cost-Effectiveness: Consider clinical and cost-effectiveness in decision-making about the organization and delivery of services.
- Assist with Office Tasks: Assist with office tasks as needed.
- Perform Additional Duties: Perform any other duties as assigned by the Medical Clinic Office Manager.
- Establish Effective Working Relationships: Establish and maintain effective working relationships with diverse individuals, including patients, family members, and other providers.
- Communicate with Patients and Family: Explain to patients and family the roles and responsibilities of each team member and how they will work together to provide services.
- Stay Up-to-Date with Healthcare Terminology: Remain fluent in terminology pertaining to a healthcare setting.
- Respect Leadership: Respect and respond to leadership displayed by providers in a healthcare setting or team.
- Facilitate Collaborative Care: Facilitate collaborative care by sharing relevant information with others through communications that are authorized by the patient and are permissible under HIPAA and related laws, regulations, and policies.
- Assess Patient's Family and Social Support System: Assess the nature of the patient's family and social support system that have an impact on healthcare.
- Ensure Information Exchange: Ensure the flow and exchange of information among the patient, family members, and linked providers.
- Use Wellness and Recovery Models: Use wellness and recovery models in conceptualizing the health and healthcare of consumers.
- Detect Signs of Abuse and Neglect: Detect signs of abuse, neglect, domestic violence, and other trauma in patients.
- Recognize Common Health Conditions: Recognize the signs, symptoms, and treatments of the most common health conditions and health crisis in a healthcare environment.
- Ensure Information Exchange: Ensure the flow and exchange of information among the healthcare consumers, family members, and providers.
- Support Patients in Considering Alternative Services: Support patients in considering and accessing complementary and alternative services designed to support health and wellness.
- Provide Information and Education: Provide information, education, guidance, and support to family and other caregivers.
- Coordinate with Health Plans: Coordinate with health plans in identifying and addressing patient and population needs.
- Promote Patient Adherence: Promote patient and family adherence to healthcare plan.
- Deliver Support in Addressing Consequences of Illness or Injury: Deliver support in addressing the consequences of illness or injury.
- Foster Diversity: Foster and value diversity in terms of the composition of the team members in all roles.
- Plan and Deliver Services: Plan and deliver services with an understanding of the healthcare needs of the population being served.
- Establish and Pursue Improvement Goals: Establish and pursue individual and team-based improvement goals.
- Screen Patients for Substance Use: Screen patients using SBIRT tool. Conduct brief, evidence-based, and developmentally sensitive screens for risky, harmful, or dependent use of substances, including alcohol, illicit drugs, and prescription medications, and appropriateness for agonist, antagonist, and anti-craving medications.
- Assess Social Determinants of Health: Screen patients using PRAPARE tool. Assess social determinants of health identifying factors that impact patient's ability to successfully follow a care plan or attend medical appointments.
- Coordinate Referrals: Coordinates all external and internal referrals to the behavioral/mental health providers identifying needs requiring additional support for BH/MH care.
- Participate in Quality Improvement Activities: Participate in Patient Centered Medical Home team meetings and quality improvement activities.
- Establish and Maintain Effective Working Relationships: Establish and maintain effective working relationships with diverse individuals, including patients, family members, and other providers.
- Participate in Data Collection and Review: Participate in data collection, review of respective outcomes reporting, as well as programmatic clinical audits and evaluation related to Patient Centered Medical Home initiatives and other quality programs as appropriate.
- Patient/Family Healthcare Practices: Knowledge/skill regarding patient/family healthcare practices across the life span.
- Effective Communication and Interpersonal Skills: Ability in the area of effective communication and interpersonal skills with public and coworkers.
- Confidentiality and Situational Awareness: Demonstrates appropriate understanding of working with confidential material and situations.
- Adaptability and Quick Learning: Ability to understand and assimilate new information quickly.
- Reporting and Data Management: Ability to accurately complete reports, gather statistics, transfer data.
- Licensure and Certification: Holds and maintains licensure as a Licensed Practical Nurse or Registered Nurse in the state of Ohio.
- Prior Experience: Prior experience working in patient/family healthcare environment preferred.
- Mental Health Knowledge: Knowledge about mental health care preferred.
- Agency Trainings: Maintains applicable agency trainings.
